What	When Needed	Comments
Registration Form	*March-August*	This gives us basic information we need about you and your child, and gets you on our list for distributing information as we get close to the season.
Release Form	*March-August*	A general liability and medical release form that also gives us emergency contact information.
Birth Certificate (an Original AND a Copy)	March-August	If you do not have an original already, contact the town or city clerk's office where your child was born and they will mail you one (they will charge you a small fee for this -- usually $10 to $15).This certificate MUST have a raised seal on it. The original certificate will be returned to you at or before the first game of the season. We keep the copy, so you should not have to submit a birth certificate again in future years. If your child played or cheered in our program last year, his/her birth certificate should already be on file. Check with the coaches or with the registrar to confirm this.
Current Physical Examination (dated no earlier than 1/1/2007)	August	We can accept one of three forms. The best is a copy of your child's most recent physical report from your pediatrician's office (it must have the office name, physician's signature, and a date on it). Alternatively, most pediatrician's offices have a standard health release form for children planning to participate in athletic sports programs -- again, it must have the office name and the physician's signature on it. Finally, if your pediatrician cannot supply either of these, you can use our form. The physician's office must stamp their office name on this form, have the physician sign it, and indicate the date on which your child was examined.
Most recent Report Card	August	We need a copy of your child's most recent report card. In addition, if your child's grade average is below 70 we require an exception letter signed by an administrator from your child's school authorizing him or her to participate. We need a letter from the school stating your child is in good academic standing. Just take this form into the office and ask someone there to sign or stamp it.
Pop Warner Academic All-Americans (optional)	August	If your child's grade average is 95 or above he or she is eligible to be recognized as a Pop Warner Academic All-American. An additional form needs to be submitted to be considered for this recognition. The Pop Warner National organization announces first and second teams of Academic All-Americans in the spring following each season. Pop Warner and leading corporate supporters provide scholarships for higher education to the most accomplished scholar/athletes. Please contact us if you would like a copy of this form.
